船舶吨税申报单

　　按照《中华人民共和国船舶吨税暂行办法》的规定，检阅有关证件（包括国籍证书、吨位证书或当地港务机关填发的证明文件），开具下列事项，请予完纳船舶吨税，并发给船舶吨税执照：

　　In compliance with the provisions of the provisionst Rcgulations Governing the Collection of Tonnage Dues of the People’：Republic of China，I hereby submit the following particulars to- gether with the rilevarn documents(imcluding the Certificate of Nationality，the Tonnage Masure. Ment Certificate or srpporting doe umetts issued by the Harbour Authorities concerned）woth the request for the issue of a Tonnage Dues C tificate upon payment of Tonnage Dues：

　　4．按三个月期或三十天期纳税（由申请人按选定划去一种）。
